鈥嬧�嬧�嬧�嬧�嬧�婽he Ontario Student Transcript (OST) is the official record of a student's secondary school academic achievement in the province of Ontario. Student records are confidential, and transcripts are issued only on the written request of the student or parent/guardian (when student is under 18 years of age) and are subject to the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA).

If the last secondary school you attended was within the 蘑菇传媒 (DPCDSB) and you require your transcript, please use the process below that best applies to you to request your Ontario Student Transcript.听聽

Applying to an Ontario College through the Ontario College Application Service (OCAS)

If you are a former student of the DPCDSB and are applying to an Ontario college through the Ontario Application Service (OCAS), request your OST using the online electronic transcript request service available through OCAS.

To use this service you must:

  • Be a past student of the DPCDSB
  • Apply to a minimum of one Ontario college
  • Pay the college application and transcript fees

NOTE:聽If you are a current student attending a secondary school within the DPCDSB, and are applying to Ontario colleges through OCAS, you do not need to request your transcript. Your transcript information will be forwarded to OCAS on your behalf. College applicants who left day school in June 2025, summer school marks and credits will be electronically transmitted and be available to OCAS and from there will be distributed to your selected colleges.鈥�

To Request your Transcript for all Other Purposes

Transcript Requests for the 2024-2025, 2025-2026 and 2026-2027 School Years:

Requests for transcripts for 2024-2025, 2025-2026 and 2026-2027 are processed by the Guidance Department of the student's current or last secondary school attended.

  1. Complete the Transcript Request Form
  2. Email the form with a legible copy of your identification to the last secondary school you attended within the board.

Email addresses for all DPCDSB schools can be found using our School Directory.听Transcripts are available for pick-up at the school or can be mailed directly to the requester.听

All Other Transcript Requests:

Requests for transcripts聽prior to the 2024-2025 school year are processed by DPCDSB's Admissions Department.

  1. Complete the Transcript Request Form
  2. Pay the applicable transcript request fees* using .
  3. Email the completed Transcript Request Form and proof of payment to [email protected].听

Transcripts will be forwarded directly to the email address of the college/university or other requested post-secondary institution to which you are applying. You will be notified once the transcript has been sent and forwarded any replies received. If you are requesting your transcript to be mailed to your home address, please include your full mailing address on the Transcript Request Form.

*Transcript request fees apply to transcripts prior to the 2026-2027 school year. Fees can be paid online through . Transcript Fees: First copy $20 CDN, additional copies $5 CDN each. Please provide proof of payment with Transcript Request Form.

Confirmation of Graduation 鈥� Diploma Letter Requests

Diploma letters for students who graduated prior to the 2024-2025 school year are processed by DPCDSB's Admissions Department.

  1. Complete the Diploma Letter Request Form
  2. 聽Pay the applicable fees* using

Diploma letters will be mailed to your home address. Please include your full mailing address on the Diploma Letter Request Form.听
